Community Habilitation

The Community Habilitation Program is designed to assist both Deaf and hearing adults and children with living independently in the community. This program aims to build skills that will increase independence in their home as well as providing opportunities for community inclusion.
Program Eligibility- Participants must be waiver enrolled in Home and Community-Based Services (HCBS) Medicaid system and be enrolled in these programs in order to participate.

About us
People with special needs require specialized support to enable them to live where they would like to live. Living with their families or in their own apartments, people with special needs often need intensive support to help them fulfill their roles in society. Mill Neck’s community habilitation program helps to level the playing field, helping people with special needs to learn, grown and access community services.
Both hearing and Deaf participants, because of the skills they have gained and through the guidance of staff members, have experienced phenomenal personal growth from the program’s inception several years ago. One of the most inspiring features of the community habilitation program is that many staff members are Deaf.

Supports include: adaptive skill development, assistance with activities of daily living, community inclusion and relationship building, training and support for independence, adult educational supports, development of social skills, leisure skills, self-advocacy and informed choice skills, and appropriate behavior development to help the individual access their community.
